A level of <50nmol/L is based on the Endocrine Society Guidelines: Researchers have used cut-off values for the suppression of serum cortisol from 100–200 nmol/L when measured by modern RIA. A widely cited normal response is a serum cortisol less than <140nml/L. Because some patients with Cushing’s disease demonstrate suppressibility to dexamethasone, use of this diagnostic criterion misclassified up to 15% of such patients as negative. Therefore, to enhance sensitivity, experts have advocated requiring a lower cut-off for suppression of the post-dexamethasone serum cortisol to less than 50nmol/L to achieve sensitivity rates of greater than 95%. |
