Notes |
Each slide must be labelled with the full patient name and MRN or DOB. NB. Use Pencil. Each St James's Hospital patient BMA request must be accompanied by an FBC sample and an Integrated bone marrow aspirate request form. Requests for Bone Marrow Aspirate examination from external hospitals must be approved in advance by the Consultant Haematologist (e.g. pre transplant marrow, second opinion) and accompanied by a Bone Marrow Examination Request Form.
