Please follow links below to the specific thrombophilia request form and the guidelines for thrombophilia testing. Testing for APCR, Factor V Leiden and Prothrombin gene mutation analysis requires genetic consent. A fully completed thrombophilia request form should be sent with samples for testing. The coagulation team can be consulted for advice for thrombophilia screen requests. Samples must be received by National Coagulation Laboratory, Centre for Laboratory Medicine and Molecular Pathology, by 4pm Monday - Friday. This is part of the Thrombophilia Screen which includes Antithrombin, Lupus Anticoagulant, Protein C, Protein S, Factor VIII, Fibrinogen and APCR. See for Thrombophilia Screen for full sample requirements. Lupus anticoagulant testing is not recommended for patient on anticoagulant therapy. Please Note: Testing for antiphospholipid antibodies should also include immunological tests for anticardiolipin and anti beta-2 glycoprotein 1 antibodies.
