- Establishing a diagnosis of Primary Biliary Cirrhosis.
Clinical Information:
- There are numerous mitochondrial antibodies which can be detected by indirect immunofluorescence. These have been classified numerically as M1 through to M9.
- The M2 mitochondrial antibody is primarily directed against pyruvate dehydrogenase and 2-oxoglutarate dehydrogenase.
- The M2 mitochondrial antibody is present in >90% of patients with Primary Biliary Cholangitis (PBC).
Please Note:
- Mitochondrial Antibody titres are not useful in the prognosis and monitoring of disease.
- All samples that are positive for Mitochondrial Antibodies on Indirect Immunofluorescence will have a Liver Immunoblot performed. This is requested by the laboratory.
- Occasionally patients with CREST (calcinosis, Raynaud’s phenomenon, esophageal hypomotility, sclerodactyly, and telangiectasia) syndrome, relatives of patients with primary biliary cirrhosis and other autoimmune diseases may have a positive Mitochondrial Antibody result.
- As the substrate (Nova Lite Rat, Liver, Kidney, Stomach Substrate Slides) in this assay is also used for the detection of Smooth Muscle Antibodies, Parietal Cell Antibodies and Liver/Kidney Microsome Antibodies, all requests for Mitochondrial Antibodies also have Smooth Muscle Antibodies, Parietal Cell Antibodies and Liver/Kidney Microsome Antibodies requested.
- The stated turnaround time is for 'normal' samples. If further testing (Liver Immunoblot ) is required the turnaround time will increase.
- The Liver immunoblot includes the antigens AMA-M2, M2-3E (BPO), Sp100, PML, gp210, LKM-1, LC-1, SLA/LP and Ro-52.
- Where External Institiutions cannot guarantee immediate delivery, samples must be centrifuged, separated and stored at 4oC. Samples can then be transported, as soon as possible, to St. James's Hospital for processing.
