Tobramycin assays are no longer performed in the Microbiology laboratory, St. James's Hospital. However Requests for tobramycin assays are referred by the Microbiology Laboratory St. James's to the Microbiology Laboratory Tallaght following discussion with the Microbiology Registrar, St. James's Hospital. Samples should be sent to Microbiology St. James's by 11:30 am , where they will be packaged appropriately and dispatched. Samples required to be sent outside of this time, must be discussed with the Microbiology Medical team. Send sample to the laboratory without delay. Sample should be refrigerated if not transported immediately There are three sample types; trough, peak and random. Trough samples are taken prior to dose. Peak samples are taken one hour post dose. Peak level: Due to limited clinical utility the Laboratory no longer processes peak levels. Samples taken at other times (Random) are of little value as they are difficult to interpret. Aminoglycosides are generally given once a day. In the presence of renal dysfunction consider alternative therapy as efficacy is concentration dependent and a large peak is therefore required. Amphotericin B may enhance the nephrotoxic effect of Aminoglycosides.
