Flow cytometry is considered the gold standard for PNH diagnosis & monitoring. It is performed by demonstrating the absence of Glycosyl Phosphatidyl anchor proteins in peripheral blood RBCs, neutrophils & monocytes which are caused by the PNH PIG-A mutation. This is done though both indirect (absence of antigen that should have been bound to the protein) and direct (absence of protein) methods. |
